Dr. Korngut participates with some insurance companies. If you have private medical insurance a request for predetermination of benefits will be sent prior to the start of treatment. Depending on your policy, there may be a co-insurance due and/or a co-pay for an office visit. Deductibles may apply. In this way, you will know what your contract covers for the therapy.
Oral appliances are considered Durable Medical Equipment (DME). The usual insurance CPT code is E0486. You can call your carrier directly for information. Please Note: Oral appliances are considered MEDICAL DEVICES and are not covered by dental insurance. Your initial consultation, dental x-rays, and models may be covered, in part, by dental insurance.
